Knowledge, the key to progress, has proven to be a human beingââ¬â¢s most powerful and significant weapon. We gain knowledge when we put our brain to work at the problems we need to solve in life. It doesnââ¬â¢t matter what we are trying to accomplish, whether it be creating a new technology or learning how to put together a puzzle, the matter of fact is that both request great examination and research to resolve and learn. Scientific research is a technique used to investigate phenomena, correct previous understanding, and acquire new knowledge. Knowledge could lead us to a possible cure for cancer, an alternative for fossil fuels, and the creation of a revolutionary technology. Nevertheless, all these benefits are a reason why John M. Barry writes about scientific research with admiration, curiosity, and passion in which he blends a use of rhetorical strategies in order to give off an overall perspective of the necessity and mystery within scientific research. Foremost, John M. Barry creates the sense of importance by describing unknown yet highly desired information that all scientists wish to obtain from scientific research, through the strategy of abstract diction where connotation is implemented at its best. The word ââ¬Å"wildernessâ⬠is referred to in the passage various times and its dictionary meaning is not whatââ¬â¢s really being discussed here. The wilderness John is referring to is the place where scientists must begin in every study in order to resolve or prove something. This is a place or in other words a moment where scientists have to take action and start guessing what is needed to do. The answer is never in the face of the scientist, and before you find it you must know where to look and how to look. Obviously, using the word wilderness creates the idea that the knowledge scientists are looking for is hard and hidden and it is a successful form of emotional appeal to characterize scientific research as secreted yet vital. Following, through a successful form of a logical appeal the writer draws an analogy that further embodies the obscurity of finding knowledge and the importance of scientific research. The author shoots to compare apples to apples when he puts side by side a shovel to the experiments performed during scientific research. When a successful comparison is given between two subjects it creates an understanding to the reader of what the overall intended purpose is for both. Both subjects compared here are meant to find an answer, or in the case of scientific research, knowledge. However, you must know what correct tool or experiment will find the answer. Itââ¬â¢s not easy, so a scientist must really put a lot of thought into what must be done and how it must be done to find the information and key data they need. In other words, the author explains how the success towards finding new knowledge is hidden deep within and can only be found if we understand step by step how weââ¬â¢re going to reach that data. Therefore, when the answer is not in the face of the questioner it takes hard effort to find but when found its benefits are unmeasured. Preceding, John M. Barry is sure not to exclude the use of an ethical appeal in which he presents a carefully and edited argument to create the strong message that scientific research is important to succeed. In the beginning of the passage, it opens by explaining what certainty and uncertainty is. Then it clarifies what it takes to be a scientist, what a scientist must do, what happens if a scientist succeeds in their work, and why a scientist may fail. The main focus of these points is based on scientific research which proves its importance in his point of view and how it can make or break a scientist. John makes it clear that scientific research is essential and is not as easy as following step by step. It takes time, dedication, and most of all determination. When someone is determined they will do whatever it takes, especially thinking out of the box, to accomplish their goal. Overall, the essay was presented in a logical and comprehensible way that allowed the reader to understand how essential yet possibly hard it can be to use scientific research. Closing, the writer is successful in making his opinion and perspective towards scientific research through the use of logos, pathos, and ethos. The overall analysis brought me to the conclusion that the John M. Barry portray scientific research as the chief ingredient to putting together answers and information. Yet still, doesnââ¬â¢t deny the complexity of scientific research and thatââ¬â¢s its not straightforward as a scientist wishes it could be. Nevertheless, the benefits scientific research has brought along we see them everyday because of our overall advancement as a world. Didnââ¬â¢t I say knowledge is our most powerful weapon? Well observe, for it has destroyed the slow and premature society humans once used to live in and created a beautiful, diverse, and intelligent culture today.

Brand association is an important dimension of brand equity because, like human association, it is both differentiating and enduring. Consumer perceptions of price, quality and value are considered pivotal determinants of shaping behaviors and product choice (Bishop, 2004). Methods of recruitment à à à à When looking at the methods to be used in recruiting people who would carry out the research, it would be prudent to seek the work of professionals. This would be because professionals are adequately equipped and have been in the practice for a while, hence would render the very best. This way value for the finances spent on the whole process is sure. A matter of great importance that cannot be ignored would be how the recruitment process should go down. This is in order to take in, not just professionals, but the best in the market. There are a number of viable and proven recruitment methods that will most assuredly give the best results, in terms of professional personality. On Fortune 500ââ¬â¢s Worst List, US Airways Group is identified as a least admired company on all eight attributes identified by the research and surveys performed by Hay Group (Best worst in, 2006). As the name implies US Airways Group operates in the Airline industry. This paper will discuss how government regulations, hard and soft technologies, some political-legal barriers, sociocultural factors and two economic theories that are faced by US Airways Group are affected by their domestic and global environments. It will also offer a strategy for US Airways Group to countinue its growth and success as an industry leader. As indicated from its name the domestic environment of this company is the United States and the global environment that will be used is Costa Rica. In the US the airline industry has two main agencies that write regulations that it has to obey. The Federal Aviation Administration (FAA) and the United States Department of Transportation (DOT) (Electronic Code of Federal Regulations, 2012). Although the airline industry was de-regulated in 1978 the DOT ââ¬Å"retains the authority to alter or amend any airlineââ¬â¢s certificate or to revoke such certificate for intentional failure to comply with the terms and conditions of the certificateâ⬠(10-K, 2012). The FAA is responsible for writing the regulations that relate to aircraft maintenance and operations, certification of pilots and flight crewmembers and air traffic and general operating rules (Electronic Code of Federal Regulations, 2012). Another other agency that establishes guidelines that the airline industry has to abide by is the Environmental Protection Agency (EPA). The EPA is responsible for ââ¬Å"regulating operations, including air carrier operations, which affect the quality of air in the United Statesâ⬠(10-K, 2012). US Airways Group has to follow all the established regulations created by the FAA, DOT and EPA if it wants to remain in business in the United States. They have to keep up-to-date with the maintenance requirements and keep accurate records as established by the FAA and continue to have their aircraft inspected at regular intervals as prescribed by the FAA (Electronic Code of Federal Regulations, 2012). They must ensure they follow EPA guidelines regarding air quality, while continuing to meet the DOT requirements for operating at efficiency or risk having their certificates revoked to do business as well. In Costa Rica, the government is based on Roman law, which is very different from the US legal system (Doing Business in Costa Rica: 2012 Country Commercial Guide for U.S. Companies, 2012). The ways the laws are written is how they are applied and there is little to no room left for interpretation by anyone regarding the laws. Therefore the way that the government regulations that US Airways Group must follow are written are the way that they must be followed. The government has established travel guidelines that must be followed there, the most important being that: currently you are not permitted to enter Costa Rica unless you have a roundtrip ticket (Costa Rica, 2012). The easiest way for US Airways to avoid an issue regarding this regulation is to ensure that all travelers going to Costa Rica have a roundtrip ticket when they board the plane in their departure city, along with valid identification. ââ¬Å"Hard technology includes the physical hardware, capital goods, blueprints and specifications, and knowledge necessary to use the hardware, while soft technology encompasses the management, marketing, financial organization, and administrative techniques that can be combined with the hard technology to serve the needs of the useâ⬠(Ajami, Cool, Goddard, Khambata, 2006, p. 358). Based on this definition some of US Airways hard technologies would be their aircraft (planes), hubs (airport locations) and even their flight crews. The planes that are currently in the fleet are manufactured by Boeing, Airbus and Embraer (US Airways Fleet, 2013) and some are equippedà with the ââ¬Å"ACSSââ¬â¢s XS-950â⠢ Mode S Transponderâ⬠which allows for the transmission of information regarding the planes ââ¬Å"position, speed and intentâ⬠(Press Releases, 2012). They have hubs in the four US cities and have established a partnership with Star Alliance (US Airways A Star Alliance Member, 2013), which gives them access to a hub at Londonââ¬â¢s Heathrow Airport as well. The flight crew has the knowledge to operate the aircraft as well operate the terminals efficiently within the hubs. It is the flight crewââ¬â¢s responsibility to ensure that the passengers are on board the plane on time and that the plane departs the gate on time. The soft technologies currently employed by US Airways includes their Nuance interactive voice response (IVR) system, which was one of their new marketing tools to help reduce customer frustrations (Interactive Case Study: US Airways, 2013), audits of customersââ¬â¢ airport experiences (Sunnucks, 2010), and training on the other new systems that have been implemented to help with customer satisfaction. Just as flight crews can be classified as hard technology the management staff of US Airways can be classified as soft technology because it is their responsibility to make sure that the customers are kept happy and that they use their administrative talents to keep the employees satisfied as well. If US Airways continues to educate, value and respect it employees, while using new technology to keep up-to-date with the rest of the industry and survey its customers for satisfaction their success will continue to improve. One of the political-legal barriers that US Airways Group will face in the US and Costa Rica the differences in the legal system as identified earlier. In the US the laws are interpreted and judges look at past cases to help decide how to rule on a current case, in Costa Rica judges look at the law the way it is written and make their rulings based on the way the law is written. If US Airways is not careful and does not fully understand the laws and how they are written in Costa Rica this could be detrimental to their operations there. In the US they not only have to comply with federal laws but state laws as well that have been established. Although both governments are classified as republics, the US has what the Central Intelligence Agency (CIA) refers to as a federal republic and Costa Rica has a democratic republic (The World Fact Book, 2012). In Costa Rica they promote a political environment based on inclusion. One of their political parties is Partido Accesibilidad Sin Exclusià ³n (PASE) which translated into English is Accessibility without Exclusion Party (Obando, 2013); however the US is not necessarily an inclusion-based society. The US does have laws regarding accessibility for individuals with disabilities and they are taken very seriously, Costa Rica is not as readily as accessible as one would think they would be based on their inclusion-based political system. For US Airways to continue to be successful in the US and Costa Rica concerning the political-legal environments within those countries they should remember the ââ¬Å"history of close and friendly relations based on respect for democratic government, human freedoms, free trade, and other shared valuesâ⬠(World-Country Profiles; US Department of State Background Note, 2007), this falls in line with the concept of the classical theory of trade (Ajami, Cool, Goddard, Khambata, 2006, p. 48). Geert Hofstede developed his theory of the five dimensions of culture, based on information provided to him by IBM regarding employee values in their survey of more than 70 countries between 1967 and 1973 (National cultural dimensions, 2013). ââ¬Å"The five dimensions of culture as theorized by Hofstede are: social orientation, power orientation, uncertainty orientation, goal orientation and time orientationâ⬠(Ajami, Cool, Goddard, Khambata, 2006, p. 213). The five dimensions are broken down into simple terms as: social orientation ââ¬â reflects a personââ¬â¢s beliefs about the importance of the individual over the group for which they belong; power orientation ââ¬â deals with how one views authority and power differences in a hierarchy; uncertantiy oreintation ââ¬â refers to how one feels regarding change; goal orientation ââ¬â deals with how one is motivated towards achieving goals and the fifth and final dimension time oreintation ââ¬â refers to the how individuals in a culture adopt an outlook of long or short term regarding life, work and other issues (Ajami, Cool, Goddard, Khambata, 2006, p. 213-214). In regards to the sociocultural aspects related to these dimensions of culture as id entified by Hofstede there are many that are different as well as some that are similar for these two countries. One of the biggest differences between the two countries is their attidudes towards time. People in the US are all about time and money whereas people in Costa Rica want to take their time and really understand what is happening. The attitudes towards work and lesiure follow suit with the attitudes towards time. Many in the US do notà take vacation time off because things need to get done, whereas Costa Ricans are more concerend with spending time with their families and socializing with friends (Provasi, 2012). The attitudes regarding change, material things, and jobs are also at opposite ends of the spectrum. These things are important to most Americans and they look at these things as a way to build themselves up. Costa Ricanââ¬â¢s on the other hand look at these things and will be just as comfortable with keeping things just they way they are. Aspects that are similar in the two countries are literacy rate and education mix. Both coutries hold these items in high regard and try to come up with ways to encourage the young people to stay in school and continue with their education and pursue higher education as well. Another item that is similar is their ââ¬Å"open doorâ⬠policy in the work place , managers are accessible at all levels and this makes it very familiar. The two economic theories that will be compared and contrasted are Walt Whitman Rostowââ¬â¢s Stages of Economic Growth and John Kenneth Galbraith Equilibrium of Poverty for the domestic and gloabal economic environments identified above for US Airways Group. The US has a mixed economic environment and is classified as a first world country. The reason the US does not fit into one specific economic environment is it has characteristics of capitalism in which the relationship of supply and demand dictates price and it also has elements of a free-market economy in which the government establishes laws and policies to ensure public safety and welfare (Ajami, Cool, Goddard, Khambata, 2006, p. 59). Costa Rica has a social market economy (Rojas, 2006) and is classified as a thrid world country (Rosenberger, 2012). A social market economy is similar to a free-maket economy in that ââ¬Å"it upholds free market components such as private property and ownership of businesses and industry, free price formation, market competition and free trade, while also utilizing government regulation to create fair market competition by preventing market monopolies, ensuring ongoing economic development and poverty alleviationâ⬠(Social Market Economy, 20 13). Based on the definitions provided it can be said the both economies are similar in regards to government regulations being involved in creating a fair and safe market environment. Walt Whitman Rostow identified five economic stages of growth in which a country could fall: traditional society, transitional stage, takeoff, drive to maturity and the age of mass consumption (Ford,à 2004). Each stage represents a stepping stone to for the next stage. According to Rostow the US surpassed the age of mass consumption in the late 1950ââ¬â¢s (Ferraro, 1960), Costa Rica however is still in the takeoff stage. This stage is ââ¬Å"marked by major transformations that stimulate the economyâ⬠(Ajami, Cool, Goddard, Khambata, 2006, p. 55). For US Airways Group, in Costa Rica this is identified by their growth in the technological area (more flights operated) as well as more jobs being created due to the increase in flights. Although in the US this stage has been surpassed, US Airways contiunes to ââ¬Å"growâ⬠in this aspect because they are increasing flight schedules. Galbraithââ¬â¢s view of economics is similar to that of Rostow, in that economic growth is as much a change in social, intellectual and policatical as well as physical (material) change (Watson, 2011). According to both of their economist views the US operates as a capitalistic economy. This is tr ue of US Airways in the way that they have been able to turn-around their business since 2005. They actually took into consideration things that would make the consumers happy, not necessarily increasing their capacity (i.e. putting more seats on a plane), in doing so they were able to keep up with the consumerââ¬â¢s wants and still provide the service that was needed (Smith, 2012). This falls into Galbraithââ¬â¢s view that ââ¬Å"the major affirmative purpose of the firm is corporate growthâ⬠(John Kenneth Galbraiths Contributions to Economics, 2013). This view is similar to Rostowââ¬â¢s age of mass consumption in that both concentrate on the high standards of living (i.e. comfort in travel on the plane) that have been established as a result of the efforts set forth is their respective stages. Where the two differ dramatically is that Galbraith believes that the companies direct the consumers wants by advertising that makes the consumers desire certain products or services, and that therefore the economy is more driven by the seller and not the consumer (John Kenneth Galbraiths Contributions to Economics, 2013). In this instance though, that concept contradicts his idea of ââ¬Ëthe major affaimative purpose of the firm is corporate growthââ¬â¢ in the example provided regarding US Airways above. In successfully industrializing a poor economy such as Costa Ricaââ¬â¢s, Galbraithââ¬â¢s Equilibrium of Poverty offers the idea of the reliable infrastructure system (Ajami, Cool, Goddard, Khambata, 2006, p. 57). This idea simply means that the country would need to have a ââ¬Å"relieableà system of roads, ports, electrical power supplies and communicationsâ⬠(Ajami, Cool, Goddard, Khambata, 2006, p. 57). In this case US Airways already has access to these items at the airport and therefore is well on its way to helping the economy grow and prosper. While both Rostow and Galbraith believe that governement need to be invovled in the initial stages to help third world countries establish themselves and help to protect their citizens they also understand that there will be a time when the government will need to step back and let the companies in the different industries takeover and control their own destinies so to speak. Rostow does not mention how or when he thinks this should be done, however Galbraith says that he feels that governments should impose ââ¬Å"taxes on consumer goods and services, using the proceeds to increase the availability of public sector goods and servicesâ⬠(John Kenneth Galbraiths Contributions to Economics, 2013). In doing this, he is trying to show that eventually there has to be a point where control is given to the companies and they only need to answer to the governments when required or necessary. US Airways Group has made significant improvements in seven of the eight-attribute areas identified On Fortune 500ââ¬â¢s Worst List 2006. On the 2011 list they still have a ranking on the worst product quality attribute of number 11, which is down from 10. US Airways Group needs to figure out ways to improve the quality of their service as well as their products. Although they may not have much control over the service aspect because the flight schedule cab be delayed because of an airline from another company they can control they products. They can invest in newer planes, upgrade/update their current planes and even update their hubs in the airports that they currently operate in. Other ways for the company to continue to grow and succeed in the domestic and global environments are they must continue to improve their offerings. They are off to a great start by taking surveys and asking customers what is important to them. They need to go a step further and ask the employees what is important to them, what will keep them loyal to US Airways Group. Take what they learn from the employees and the implement changes that are suggested. Continue to ask the customers what they like, do not like, what other features they would like to see in the airports, on the planes and in the air. I think one thing that the company should do is take a lesson from Southwest and stop charging baggage fees for the first checked bag. They should eliminateà change fees, and if someone needs to cancel their trip completely, implement a cancell ation fee, but refund the difference between the ticket price and the cancellation fee. By eliminating their fees they will draw more passengers to the airline and increase sales, because people will not feel as if they are being taken advantage of. In todayââ¬â¢s economy this will go far with consumers and could lead to even bigger profits in the end, because they will feel that US Airways Group is a company that is not just looking out for themselves but for their customers as well. As demonstrated there are several different environments that a multinational corporation must take into consideration when exploring new opportunities. The government regulations, hard and soft technologies, political-legal barriers, sociocultural factors and economic theories that a company faces and how they handle each of those environments will determine if they will succeed or fail. Walt Whitman Rostow and John Kenneth Galbraithââ¬â¢s economic theories can be used by companies to help determine if the global environment that is being considered will be a good fit with the domestic environment of the parent company. Once the location has been setup and establishe the company must realize that the work does not stop there, they must always analyze and remember they want to continue to grow so they need to ask the question what do we do to contiune to prosper or what do we do to make it so we do prosper. They are basically saying that every calorie counts and not just the ones in their products. Bittman describes this video as, ââ¬Å"Sheer manipulation, calculated to confuse, obscure and deny. â⬠Bittman beliefs sugar, ââ¬Å"Especially in liquid formâ⬠is extremely harmful and not just because it leads to obesity. He thinks Coca-Cola is more focused on making money than it is trying to help the nation deal with obesity. He goes on to compare soda to nicotine saying, ââ¬Å"Soda is a fructose delivery system as tobacco is a nicotine delivery system.â⬠He is stating that Coca-Cola is doing the same thing tobacco companies once did, ignore the fact that their product was extremely harmful and they didnââ¬â¢t want the consumers to know because it would lead to a loss in product sales. This article focuses on the negative effects that sugar based beverages have on the body. Bittman states, ââ¬Å"There is virtual consensus that drinking too much soda is bad for you, and itââ¬â¢s not hard to understand the evidence.â⬠In the end Coca-Cola is simply trying to lessen its bad reputation for causing health problems such as diabetes and chubbiness by releasing a video which talks about obesity. Bittman is implicating the irony in Coca-Colaââ¬â¢s video and he simply trying to warm people not to drink soda because itââ¬â¢s not healthy.

Effectiveness Impact of Journal Clubs in Medicine Abstract Background: Journal clubs` emerging goals are now considered to be teaching critical appraisal skills and learning how to use evidence based medicine in practice. Although journal clubs are well accredited, designing the right format to keep members stimulated and educated remains a great challenge. Methods: We conducted journal club structure modifications in internal medicine residency program of a university affiliated hospital. Initially group-based sessions identified feasible changes and baseline data concerning residents` knowledge of evidence based medicine (EBM) was collected by a questionnaire. Modifications implemented and a second set of group discussions and data collection took place after 12 months. Results: Total number of 78 residents participated. The most important identified changes were schedule adjustments and setting new goals regarding EBM, medical statistics and critical appraisal teaching. Group discussion showed increased satisfaction and questionnaire assessments showed significant improvement in residents` knowledge of EBM. Conclusions: Redesigning journal clubs with emphasis on regularity and setting new horizons basically improves their effectiveness. Assigning entire sessions to augment participants` skills in new areas of knowledge is a unique way to fit journal clubs as a novel and innovative teaching practice. Practice Points Journal clubs are connecting bridges between knowledge and practice. Journal clubs have gradually evolved to be a teaching tool in modern medicine. Considering growing popularity of evidence based medicine, journal clubs can be a way to introduce this discipline into educational programs. Redesigning old journal club structure is somehow necessary to better keep up with growing knowledge of medicine. Furnishing journal clubs with innovative methods might be a reasonable way of reform. Introduction The earliest reference to the origin of the phrase journal club is in memoirs and letters of Sir James Paget, describing a small room near St. Bartholomews Hospital in London in the period 1835-1854 in which pupils met and read journals (Paget 1901). More than a century has passed since Sir William Osler started the first recorded journal club in North America in 1875 as a way of sharing periodicals he could not afford individually and later established a club at Johns Hopkins University to review the latest medical research (Alguire 1998; Sleeman 1990). Todayââ¬â¢s journal clubs have evolved a great deal and are found in nearly every medical school and residency training program in almost all fields of medicine (Valentini Daniels 1997; Sidorov 1995; Green 1999). Traditionally journal clubs are educational meetings in which a group of individuals meet to discuss and critically evaluate the current articles in the scientific literature (Mcleod et al. 2010; SVN Research Committee 2009). The major aim of journal clubs was to keep their attendees up to date with the latest medical literature (Valentini Daniels 1997; Forsen 2003; Goodfellow 2004). Gradually they have become a means for teaching critical appraisal skills, improving biostatistical knowledge, getting familiar with epidemiologic methods and most recently promoting the practice of Evidence Based Medicine (EBM) (Valentini Daniels 1997; SVN Research Committee 2009; Woods Winkel 1982). Critical appraisal skills and basing clinical decisions on the best published evidence available (i.e. EBM) have become an important facet in clinical medicine and are part of core general competencies required by the Accreditation Council for Graduate Medical Education (ACGME) in all residency programs (ACGME 2009; Yew Reid 2008; Carpenter et al. 2010). In the ensuing years, EBM has enjoyed widespread popularity. Todayââ¬â¢s journal clubs are effectively assisting practitioners translate knowledge into practice and serve as an excellent bridge between research and clinical work. While journal club has been a mainstay in postgraduate medical education for many years, several authors have pointed out the diversity of its format (Alguire 1998; Sidorov 1995; Ebbert et al. 2001). The desirability of journal clubs in internal medicine has been reinforced by accreditation requirements that mandate residents` participation in journal clubs. Although the goals and purposes of journal clubs are well established, selecting the right format and setting to keep members stimulated and educated remains a great challenge (Kelly Cronin 2010; Hartzell et al. 2009). With the growing appreciation of evidence based medicine (EBM) and its widespread application in clinical decision making, we thought of revising and implementing possible modifications in our journal club structure to improve quality of sessions and weigh their effects on our journal club program. Methods We conducted revision program into our journal club structure involving internal medicine residents attending the internal medicine residency program in a teaching university affiliated hospital. The modification process consisted of several steps. First we conducted group-based discussion sessions with participation of two clinical experts, two research experts, the program manager and chief resident. Group discussion primarily consisted of brain storming, reviewing similar works and their outcomes and sharing experiences. Five such meetings were held with main purpose of gauging current program features and identifying possible essential changes required to improve the program. Findings of these sessions then were shared with residents` delegates, 2 residents from each year, through three separate sessions. Identified feasible modifications were finally determined out of group-based discussions and implemented to make a new journal club structure. The group-based discussion took place for the second time, 12 months after executing alterations. Group participants were the same. This time the group assessed different aspects of changes made including residents` attendance, residents` satisfaction and the content of new sessions. Furthermore we measured the residents` existing knowledge of EBM by a self-reported questionnaire, designed and internally validated by the discussion group. The questionnaire consisted of three questions to assess three fields: residents` knowledge of statistical significance, their acquaintance with evidence rating and their familiarity with study design. Possible answers were as ââ¬Å"No familiarityâ⬠, ââ¬Å"Fair familiarityâ⬠, ââ¬Å"Good familiarityâ⬠and ââ¬Å"Excellent familiarityâ⬠. In order to facilitate further comparison, the first two answers were classified as cluster 1 (limited knowledge) and the two remaining answers as cluster 2 (acceptable knowledge); therefore making comparison between two clusters. Each resident was given the questionnaire before applying changes and again 12 months after running the new program. Residents` responses were also discussed in the second set of group discussion. The results were collected anonymously and data was then imported to Excel software (2010, Microsoft) for examination. We specified the distribution of answers in each cluster before and after modifications and calculated the absolute difference between them using descriptive statistics. Results Seventy-eight internal medicine residents (43 Female, 35 Male) were included into the survey. An almost even participation was observed among residents from all years of internal medicine training. Results of this study can be divided into two phases; before applying changes and after implementing modifications. Phase I Founded on the primary group-based discussions, the acknowledged key points for developing our new journal club structure encompassed program and schedule modification, adding motives, defining new goals, content revision and implementing feedback mechanisms from which, content revision was considered the most critical one. The main aspects of modifications made are explained in details. Our new schedule was designed as weekly sessions on Thursdays (the day before weekend in our country). We planned the sessions to be held from 7 to 8 in the morning (Breakfast time) and on the day with minimal educational and clinical workload to assure maximal attendance. To ensure the regularity and predictability of our program, we already scheduled the sessions for the next entire year. Holidays, exam days and residents` rotations were carefully noticed in planning the journal club calendar and the presenter for each session was specified at the beginning of the educational year. Furthermore we changed the place to a bigger room with better facilities. Also we decided to provide a mini breakfast meal during all sessions as a potential incentive that can increase participants` interest. Defining new objectives was also emphasized by group members. Therefore, we set our new goals not only to keep updated with medical literature and review the current literature, but also to introduce medical statistics to residents, encourage arguments and develop critical appraisal skills and understanding EBM. We changed the composition of our journal club participants to include relevant attending physicians, medical statisticians and experts in EBM and critical thinking as well as internal medicine residents from all levels. In order to achieve the broad goal spectrum, our reformed program consisted of two different types of sessions. Three sessions of each month were dedicated to presenting review articles and discussion about the main topic (analogous with former program). The last week of the month was devoted to evidence based journal clubs in which an original article was presented to the audience and 2 to 3 relevant articles or guidelines were also considered. The main focus of these sessions was to discuss the methodology and to point out statistical points through which critical appraisal skills were practiced and evaluation of the quality and validity of the results taught. Both types of sessions were moderated by an attending physician with experience in medical research and education background. Review article journal clubs were presented by a second year internal medicine resident employing slideshows; hence each resident was provided the opportunity to benefit from this chance at least twice a year. Evidence based journal clubs were presented by senior residents familiar with medical research and statistics and with acceptable teaching skills. These sessions were supervised by a team (at least 2 persons) of expert physicians in EBM and critical thinking skill. A support committee consisting of two senior internal medicine residents, journal club moderator and an EBM expert was formed for evaluating and choosing articles for presentation. The responsible resident had to provide up to 5 papers to the committee, who would then return one or two of them for presentation. The major selection criteria were relevancy and novelty of the topic and being in accordance with the interests of the participants. The other thing considered especially for articles to be presented in EBM sessions was being somehow controversial regarding the methodology or conclusion. At the end of each session a brief explanation was provided including critical points and a conclusion. For EBM sessions the strength and limitations of the article were also emphasized, inspiring the participants to involve in this process valuable in didactic endeavors. The conclusion of each session was then uploaded in the departmental forum of residents and interns for more discussion. Sometimes, a practical question was also provided in order to ascertain that residents have got the point correctly. Phase II Second phase results are described after 12 months of running the new program. Group-based discussion findings for new schedule clearly showed increased attendance and residents were more satisfied with the fixed and pre-defined sessions. Nonetheless we noticed that providing food was not of residents` concerns. Definition of new objectives for journal club sessions persuaded residents to participate more. Experts in the group-based discussions were also satisfied with the new content of sessions and monthly devoting an entire session to EBM and teaching statistical points and critical thinking. Regarding questionnaire assessment, final results showed that in the field of ââ¬Å"evidence rating knowledgeâ⬠, the number of residents in cluster 1 reduced from 51 to 17 and accordingly the number of residents in cluster 2 increased from 27 to 61 (43.6% absolute change regarding the total number of participants; P Similarly, before-after comparison for the field of ââ¬Å"statistical significance acquaintanceâ⬠revealed the absolute change to be 23.1%; i.e. the residents` acquaintance with concept of statistical significance was noticeably augmented, changing from limited knowledge to acceptable knowledge, in 18 residents (P Discussion Our conventional journal club structure first started in internal medicine department of Shariati hospital, Tehran University of Medical Sciences in year 2007. Since then it was running in a small room and on a weekly basis with focus on review article presentation only. Participants were mainly residents, internal medicine interns and medical students and presenters of sessions were selected from first year residents. There are considerable studies describing journal clubs conducted in different settings and for different health providers; yet there appear to be no ââ¬ËGold Standardââ¬â¢ approach for conducting a journal club or assessing its effectiveness. It is therefore an ongoing challenge in designing effective journal club format that assists the participants to translate journal club activities into evidence based practice (Deenadayalan et al. 2008). As residency programs deal with work hour restrictions and implement competency educations (Hatala et al. 2006; Goroll et al. 2004), they need to undergo intense reassessment and possible remodeling to determine if they are meeting their goals. Our questionnaire survey showed that our reformed program made significant change in residents` knowledge and the number of residents significantly shifted from low knowledge to acceptable knowledge. We postulated that holding journal clubs on a regular basis is necessary to achieve continuous educational progress and gradually obtain the desired improvement in residents` competency which is in accordance to most previous studies (Deenadayalan et al. 2008 ). Regularity of sessions basically show the importance of this type of learning, as a new method of education is being introduced to the learners (residents). On the other hand, disorderliness might suggest that the program is not so useful and coordinators are not sure of its value, which in turn will dissuade participants from joining and following the schedule. Pre-defining the whole schedule would be a great way in achieving this goal. Although fixed schedule might seems impractical for many institutes but authors assumed that accurately considering some fine points such as work hourââ¬â¢s prerequisites before scheduling the program may be helpful. In addition, this would assist participants to adjust their time and for presenter to know their schedule and being prepared for that. Regarding best frequency, according to most of studies reporting this, it seems that monthly period is endorsed (Letterie Morgenstern 2000; Burstein et al. 1996). Our reformed structure had a weekly basis, though our new EBM sessions were held monthly, not to shrink residents` enthusiasm for participating in these educational gatherings. Some studies have shown that timing of journal clubs might be of importance considering attendance, especially when they were coincided with meal times (Bazarian et al. 1999; Langkamp et al. 1992). While they advised provision of food as a way to increase attendance, our group-based discussions and residents feedbacks did not show such relation. This might be due to . Authors believed that forward movement without adding new goals and/or lacking enough motivation would not be successful in improving participants` competency. Increasing knowledge of medical statistics and understanding EBM were the primary new objectives of our program which is demonstrated to be well achieved regarding our survey. Participants of journal clubs must be in accordance to the aims and contents of the program. Similar to Hartzell (2009), authors think that inviting attending physicians and involving senior residents may contribute to fulfillment of journal club objectives. This composition would facilitate the arguments and give space for better discussion and reaching a practical point. Assigning an entire separate session to introduction of this new area would also be of great benefit, since innovative thinking and learning new skills necessitates more time. New educational goals for residency programs now include conduction of problem oriented sessions, evidence based journal clubs and also experiencing critical appraisal. Considering that almost all of medical education programs have introduced journal clubs as a routine in their curricula, we thought of it as a useful tool to familiarize our residents with better interpreting medical literature, critical thinking and evidence based medicine. This approach has been supported by previous researches (Hatala et al. 2006; Ghali et al. 2000). Although this survey showed promising results and improvement in residents` knowledge and participation, our results are far from perfect. Yet we must increase our efforts to recognize subtle obstacles and further improve effectiveness of journal club. Conclusion In conclusion, journal clubs are of great value in today`s medical education addressing both clinical practice and evidence based learning improvement. Yet there is no gold standard to achieve the best result with this regard. We have enjoyed revolutionizing our traditional structure of journal clubs and furnishing it with novel objectives and construction and noticed significant improvement in attendance and competency of our internal medicine residents. Salmon farming represents one very important way to feed the world and people want to eat more salmon and seafood- more than can be caught. Salmon farming began over 30 years ago and has become a huge industry. Experts say itââ¬â¢s the fastest growing segment of agriculture. Salmon farming plays an important role in the economies of many areas as well. Jobs and other economic benefits contribute to the value of salmon as much its role in good nutrition. Salmon is an oily fish rich in omega-3 fatty acids, a substance that almost certainly helps protect against heart disease and may also reduce the risk of cancer and Alzheimer's. There is one species of Atlantic salmon and five species of Pacific. Atlantic salmon account for almost 95 percent of the farmed salmon produced, and most of them are farm-raised on the pacific coast. Pacific species account for all of the wild salmon caught in the Americas and some of them are also farm-raised. No wild Atlantic salmon are fished commercially in North America, as they are an endangered species. Atlantic salmon have become the species of choice to raise on farms because they are more adaptable to the farming techniques and make better use of feed so they produce more salmon with less feed. Not everybody agrees however, that farmed salmon raised in net pens are healthy for the environment or for you to eat. Over the years, there have been numerous stories in the media that have pointed out the negatives of farm raised salmon. After his mother died, his maternal grandmother granted two London merchants, John Rowland Sandell and Richard Abbey, guardianship. Abbey played a major roll in the development of Keats, as Sandell only played a minor one. These circumstances drew him extremely close to his two brothers, George and Tom, and his sister Fanny. When he 15, Abbey removed him from the Clarke School, as he became an apothecary-surgeon’s apprentice. Then in 1815, he became a student at Guy’s Hospital. He registered for a six- month course to become a licensed surgeon. Soon after he decided he was going to be a doctor he realized his true passion was in poetry.
